Nerf League
It is important for you, as the parent/guardian to read each section through, in its entirety. Below each section is a space for you to type out your full name, which is your digital signature. Your digital signature constitutes that you have read, understand, and agree to the information above. In exchange for allowing my child to participate in this church-sponsored activity and/or event, I hereby assume all risks of injury and/or damages of whatever type or form associated with my child’s participation in this activity and/or event. I understand that my consent on this form also constitutes my consent for Pastor David Greenidge and/or a volunteer of Calvary Pentecostal Tabernacle to consent to medical care providers diagnosing and providing medical treatment to my child, at my expense, in the event of injury or illness requiring emergency or other medical treatment while involved in this activity and/or event, or associated with this activity and/or event. I waive any claims or causes of action, including attorney’s fees, I might have against Calvary Pentecostal Tabernacle, Pastor David Greenidge, and/or Calvary Pentecostal Tabernacle volunteers for allowing my child to participate, and also against anyone who provides medical treatment to my child in reliance upon this agreement. I agree to hold Calvary Pentecostal Tabernacle and its associates harmless in the event they provide medical treatment or are subsequently sued for injuries to my child on this activity and/or event.
This program is exclusively for children who have entered in grade 2 to grade 6 at the start of the school year in September. Children who are outside of this age rage are not allowed to participate in Nerf League. If it is discovered that your child is not in these age parameters and is attending Nerf League they will be asked to leave immediately. There is also a $2 fee per night, per family to help cover the cost of equipment throughout the season. Siblings who are within the age parameters are covered in a single $2 attendance fee.
This year we will be using a yellow card/red card system as disciplinary measures for inappropriate behaviour (see list below). Should any incidents occur, cards will go home with a child and will be documented. Children who participate in inappropriate behaviour, physically and/or verbally will be given a yellow card, if their behaviour is not corrected after a warning. After receiving 2 yellow cards, children will be given a red card, expelling them from Nerf League for 1 weeks time. When a child returns, if the behaviour is not corrected, a child will receive a second red card and be expelled from Nerf League for 1 month. If the behaviour continues after that, children may be asked to not return to Nerf League for the remainder of the school year.
Inappropriate behaviour includes but is not limited to:

swearing
-
bullying
-
hitting, punching and kicking

inappropriate use of Nerf equipment

refusal to follow Nerf League rules and guidelines
