Extra Life 2024 Registration

8:00am Friday October 25th to 8:00am Saturday October 26th at Calvary Pentecostal Tabernacle, 375 Westmount Drive N, Orillia, ON L3V 6Y4.

The following information is for the purpose of informing the parent/guardian of the following activity and/or event that their student desires to partake in, and requesting the parent’s/guardian’s consent and release to the care and supervision of Pastor David Greenidge and volunteers of Calvary Pentecostal Tabernacle. Please read this document in its entirety before signing.

Extra Life is a 24-hour gaming marathon fundraiser that raises money for Sick Kids hospitals. Come join us for a fun event with video games and board games.

To participate in Extra Life, this registration form must be filled out and returned to Calvary Pentecostal Tabernacle by Monday October 21st.

We encourage participants to sign up for Extra Life at https://www.extra-life.org/ and ask friends and family for donations. You may sign up as an individual or you can join our team (CPT Extra Life). Plan on bringing or arranging food as meals are not provided.
