Parental Agreement

It is important for you, as the parent/guardian to read each section through, in its entirety. Below each section is a space for you to type out your full name, which is your digital signature. Your digital signature constitutes that you have read, understand, and agree to the information above.  In exchange for allowing my child to participate in this church-sponsored activity and/or event, I hereby assume all risks of injury and/or damages of whatever type or form associated with my child’s participation in this activity and/or event. I understand that my consent on this form also constitutes my consent for Pastor Jaclyn Jeans and/or a volunteer of Calvary Pentecostal Tabernacle to consent to medical care providers diagnosing and providing medical treatment to my child, at my expense, in the event of injury or illness requiring emergency or other medical treatment while involved in this activity and/or event, or associated with this activity and/or event. I waive any claims or causes of action, including attorney’s fees, I might have against Calvary Pentecostal Tabernacle, Pastor Jaclyn Jeans, and/or Calvary Pentecostal Tabernacle volunteers for allowing my child to participate, and also against anyone who provides medical treatment to my child in reliance upon this agreement. I agree to hold Calvary Pentecostal Tabernacle and its associates harmless in the event they provide medical treatment or are subsequently sued for injuries to my child on this activity and/or event. 

These program is exclusively for students who have entered in grade 5 to grade 8, and grade 8-12 at the start of the school year in September. Students who are outside of this age rage are not allowed to partake in the desired program. If it is discovered that your student is not in these age parameters and is attending one of these programs they will be asked to leave immediately. There is no fee for regular ministry nights. However throughout the school year there may be special event nights and retreats on and off the church property which will typically have a cost. Should your student desire to participate in said events and/or retreats they will be required to pay the appropriate amount in order to attend. Pastor Jaclyn will give advanced notice to you and your student of when these events are approaching, and will ensure that the appropriate consent and release forms are sent home.

Your student's safety and well-being is our top priority while a part of our programs. With this in mind, it is up to the discretion of Pastor Jaclyn that certain behaviour warrants a student to be sent home. If this is the case, parents/guardians will be notified, and will be asked to pick up their student unless prior transportation has not be previously arranged (i.e. student is granted permission from parents/guardians to walk home).

Inappropriate behaviour includes but is not limited to:

  • vandalism

  • bullying

  • hitting, punching and kicking

  • kissing and inappropriate touching

  • refusal to follow rules and guidelines
